Initial Assessments
Initial assessments are for new patients. These visits are scheduled for 60 minutes and are designed to better understand current concerns, symptoms, functioning, and goals for care in a calm and supportive environment.
Appointments may include discussion of emotional, behavioral, attentional, or mood-related concerns, relevant history, previous treatment experiences, and recommendations for ongoing support and treatment.
How can I prepare for my initial assessment?
Please complete all new patient paperwork and recommended assessments prior to your appointment, bring a list of current medications, and arrive 15 minutes early for your scheduled visit.

Medication Follow-up Visits
Medication follow-up visits are ongoing appointments focused on monitoring symptoms, treatment response, medication effectiveness, side effects, and overall progress.
These appointments typically last approximately 30 minutes. Follow-up visits commonly occur approximately 1 month after a medication change is made and every 3–6 months for ongoing maintenance, depending on individual needs and treatment recommendations.
The visits are often paired with supportive psychotherapy, which allows time to discuss emotional functioning, coping skills, stressors, behavioral patterns, and overall wellbeing while supporting continued progress outside of medication management alone.
How can I prepare for my follow-up visit?
Please complete any requested assessments prior to the visit, bring an updated list of current medications, be prepared to discuss symptoms, concerns, side effects, and progress since your last appointment, and arrive on time for your scheduled visit.

We Offer A Modern & Informed Approach To Your Mental Health Treatment.
We offer pharmacogenomic and genetic testing services, including GeneSight® and GeneDx®, when clinically appropriate to help guide treatment planning and diagnostic clarification. These non-invasive tests are typically completed using a simple cheek swab and may be covered by insurance depending on individual benefits and clinical criteria. No. Treatment recommendations are individualized. Some patients may benefit from therapy, behavioral interventions, school supports, lifestyle changes, parent support, or additional evaluations without medication.How involved are parents/guardians in treatment?
Parent and guardian involvement is often an important part of child and adolescent psychiatric care. Fog & Fern Psychiatry also works to support developmentally appropriate privacy and trust for adolescents whenever clinically appropriate and legally permitted.Can you communicate with my child’s school or therapist?
Yes, when clinically appropriate and when proper consent forms have been completed. Collaboration may help support treatment planning, academic functioning, behavioral support, and continuity of care.What happens during the first appointment?
Initial evaluations are typically more comprehensive than follow-up visits and may include discussion of:

Follow-up frequency varies based on clinical needs, medication monitoring, safety considerations, and treatment recommendations. Some patients may require more frequent visits than others.What if my child is in crisis?
Fog & Fern Psychiatry is an outpatient practice and does not provide emergency psychiatric services, crisis stabilization, or after-hours emergency care. In an emergency, call 911, contact 988, or go to the nearest emergency room.How do medication refills work?
Medication refill requests should be submitted at least 5 business days in advance whenever possible. Controlled substances and certain medications may require follow-up appointments prior to refill approval.What if we miss an appointment?
Missed appointments, late cancellations, or repeated inconsistent follow-up may result in fees or impact the ability to continue treatment safely and effectively. Please review the cancellation policy included in your intake materials.Do you offer telehealth appointments?
Yes. Telehealth appointments may be available when clinically appropriate and when patients can participate safely from a private location using secure HIPAA-compliant technology.What should we bring to the first appointment?

Fog & Fern Psychiatry currently offers limited and flexible scheduling availability, as the provider also maintains a separate full-time position. Appointment availability may vary from week to week and is offered by scheduled appointment only. Every effort will be made to accommodate patients and families whenever possible.
